Lost Student ID Replacement Form

Student ID cards are an important part of school safety and daily operations. They are required for identification, access to certain areas of the building, participation in school activities, and use of services such as the cafeteria and library.

Each student is issued one free ID card per school year. After that, replacement ID cards cost $7.00 each. Payment may be made online or in cash to the WTB Main Office.

Please complete this form to request a replacement. Once payment (if required) has been received, your new ID card will typically be produced within 72 hours. You will be notified when and where to pick up your replacement card.
